MDS Coordinator (RN or LPN)

WHO WE ARE: Arbor Springs Health & Rehabilitation Center, a 180 bed Skilled Nursing Facility in Ocala, FL is currently seeking an MDS Nurse.  We are a growing healthcare facility in need of an energetic, motivated, and talented individual to join our team!

As the MDS Nurse within a Skilled Nursing Facility you will be responsible for the coordination and completion of the resident assessments, and overall support of the clinical staff.  Here @ Arbor Springs Health & Rehabilitation Center, we believe in a strong TEAM approach that truly improves the lives of all our residents.  

MDS Nurse Job Summary & Qualifications:

• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Verify all MDS documents 
• Ensure Care Plans contain problem, goals, and interventions.
• Active RN License Preferred (may also take LPN if the below requirements are met in full)
• Minimum 3 Years MDS Experience in LTAC, SNF, or Hospital setting
• Advanced Organizational Skills
• Advanced Conflict Resolution Skills
• Must have a current/active CPR certification.
• Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
• Self-motivated to learn and master the RAI Process in an effort to attain or maintain the patient’s highest practical level of function.
