Position Summary:

At CareMore our Mental Health Therapist is responsible for providing general psychological care and treatment to patients in medical facilities such as the clinic, health center, SNF's, Long Term Care Facilities or patient homes.

The Therapist (LCSW) must possess efficiency in creating and maintaining Electronic Health Records, efficiency in utilization of electronic media for communication., and must be a ble to manage psychotherapy caseload as well as function within BH team and medical model to prevent hospitalizations when possible.

You are expected to p erform individual and group psychotherapy as well as conduct telephonic sessions, and p articipates in high risk patient care and planning both for traditional medical and Behavioral Health members, function in a fast paced Integrated Behavioral Health model, and conduct crisis interventions.

Primary duties may include, but are not limited to:
Performs mental status examinations, therapy, and preventive health measures. Reviews psychological diagnostic tests to identify and assess patient's clinical problems and health care needs. Records physical findings, and formulates a plan and prognosis, based on the patient's condition. Discusses case with the Physician and other health professionals to prepare a comprehensive patient care plan. Submits the health care plan and goals of individual patients for periodic review and evaluation by the Physician. Administers or recommends individual therapy or other forms of treatment. Refers patients to the Physician for consultation or specialized health resources for treatment. Performs individual and group psychotherapy as well as conduct telephonic sessions Participates in high risk patient care and planning both for traditional medical and Behavioral Health members

Requires a MS in Psychology or equivalent field with a minimum of 2 years of experience; or any combination of education and experience, which would provide an equivalent background. Experience in providing mental health services within a geriatrics environment; preferred. Bilingual Spanish/English a plus.

Current Licenses/Certifications: Marriage and Family Therapist (MFT) or License Clinical Social Worker (LCSW) or equivalent in good standing with the applicable state required.

Must have reliable transportation. Willing to travel throughout multiple counties.
