Job Description

AACF is a nonprofit organization that seeks to enhance, empower and enrich our community through outreach and support. We seek to enable community members to increase their capacity while understanding and appreciating the different cultures among those we serve.

We are looking to hire individuals who care about the community and seek to make a difference. We offer paid training in exchange for your commitment to help serve Frederick and Washington Counties. Our programs vary based on community needs and focus on education, health and wellness.

Our CHWs will be asked to assist community members and their families to navigate and access community services, resources, and adopt healthy behaviors. The CHW supports providers and individuals through an integrated approach to care management and community outreach. As a priority, all activity will promote, maintain, and improve the health of patients and their family. CHWs provide social support and informal coaching, advocate for individuals and community health needs, and provide services they will receive training for. Community outreach, such as visits in the home and health screenings may be required. Community Health Workers come from the communities they serve, working at the grassroots level building trust and vital relationships which make them effective culture brokers between their own communities and our systems of care.


Key Duties and Responsibilities:


  • Responsible for establishing trusting relationships with individuals and their families while providing general support and encouragement.

  • Providing ongoing follow-up, basic motivational interviewing and goal setting with individuals/families.

  • Conduct intake interviews with individuals, including enrolling and/or referring patients into services and programs.

  • Follow-up with individuals via phone calls, home visits and visits to other settings as identified.

  • Assist individuals with completing applications and registration forms.

  • Help individuals set personal goals, and attend appointments.

  • Provide referrals for services to community agencies as appropriate.

  • Help individuals connect with transportation resources and give appointment reminders in special circumstances.

  • Exhibit excellent working relations with patients, visitors and staff, effectively communicating the mission of the AACF.

  • Work closely with providers to help ensure that patients have comprehensive and coordinated care. Follow-up with patients should be continuous from initial identification through closure.

  • Work cooperatively with other personnel assigned to the same individual.

  • Be knowledgeable about community resources appropriate to needs of individuals/families

  • Be responsible for providing consistent communication

  • Coordinator to evaluate individual/family status, ensuring that provided information, and reports clearly describe progress.

  • Act as a patient advocate and liaison between the individual/family and community service agencies (i.e. schools, physicians, home care, hospitals, support groups, etc.).

  • Record individual care management information no later than 24 hours after patient contact.

  • Attend regular staff meetings, trainings and other meetings as requested.

  • Manage assigned caseload of patients.

  • Other duties as assigned.


Minimum Qualifications:


  • High School Diploma or its equivalent.

  • Successful completion of a Community Health Worker formal training program provided by AACF

  • Written and oral fluency in English required.

  • Basic computer skills.

  • American Heart Association BLS Healthcare Provider - not required but preferred

  • Ability to initiate and maintain positive working relationships with staff and other organizations.

  • Understand the community served - community connectedness.

  • Good communication skills, such as listening well, and using language appropriately.

  • Ability and willingness to provide emotional support, encouragement and motivation to individuals
