Job DescriptionJob Title: Registered NurseReports to: Charge NurseFSLA Status: Non-ExemptPOSITION SUMMARYThis position is responsible for assisting the Charge Nurse with the daily oversight and management of the client services at Alsana. This position includes work in the areas of staff development, client admissions and discharges, medication management, medical procedure oversight, medical documentation review and oversight, on-going clinical staff supervision, training and compliance, and other duties assigned by the Director of Nursing. This position requires knowledge of team building and an ability to use persuasive leadership in order to work with others to ensure delivery of safe and effective clinical services.
This position requires working knowledge of federal, and State laws.The Joint Commission standards as they pertain to client care in a congregate living health facility. Delegates tasks to direct care staff including but not limited to vital signs, grooming, dining supervision, close observation when needed, etc. and ensure all tasks are completed. Assesses client health and well-being daily.
Promptly reports concerns to the physician and other team members who need to know and documents all findings and interventions. Communicates pertinent nursing and care plan-related information to direct care staff on a daily basis. Ensures "hand-off" communication each shift.
Participates in treatment team meetings and contributes to the client care plan on a regular basis. Participates in developing and maintaining the client's treatment plan. Participates in client education and leads groups as assigned.
Coordinates new admissions: Completes the nursing assessment within 24 hours. Completes a suicide assessment on admission and when indicated. Creates a new client entry on the weights and vitals spreadsheet and document updates.
Organizes all paperwork that is generated upon new admission, including new labels, new file folders for all paperwork, stickers for clients, discharged paperwork, sections in the MAR, and client folder for physician order book. Completes Weekly Nursing Update Assessment in Sigmund: Medication orders input into the electronic medical system. Enters client data for new admissions e.g.
height, weight allergies, etc. Verifies the order with the computer input. Verifies the MAR with the MD order.
Ensures a clinical reason is entered for each medication. Verifies that client consent is received for the psychotherapeutic drugs. Ensures that no unapproved abbreviations are used.
Supervises physician telephone or verbal orders: Ensures "read back" documentation for telephone or verbal orders. Confirms a clinical indication for each medication ordered. Creates a new MAR from physician order.
Orders initially prescribed medications and processes all physician medically related orders. Ensures all allergies of each client on the MAR and on the chart. Supervises medications pass: Checks client's medications against the physician orders every 24 hours.
Ensures that nursing personnel checks two forms of ID prior to administering medications. Maintains the medication cart with clean supplies. Changes water etc.
after each medication pass. Destroys wasted or discontinued medications according to Alsana policies and procedures. Audits and monitors all controlled substances including the recording of information.
Ensures that all facility medication policies and procedures are followed. Ensures that the physician obtains informed consent and nursing reinforces education prior to administering psychotherapeutic medications.
