Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, client/family, and referring agency; and assumes the responsibility for coordination of care. Job Classification: Clinical Lines of Authority: Reports to Director of Clinical Services Job Qualifications: Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure: Current Texas State license as a Registered Nurse, current Texas Drivers License Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, home health preferred.

Proof of current CPR, negative TB screen, and Hepatitis profile. Transportation: Reliable transportation. Valid and current auto liability insurance Environmental and Working Conditions: Works in clients' home in various conditions; possible exposure to blood and body fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call. Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50 lbs. and move clients. Requires working under some stressful conditions to meet deadlines and client needs, and to make quick decisions and resource acquisition; meet client/family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity. Essential Functions:
Initial and ongoing comprehensive assessments of home health client's needs, including OASIS assessments at appropriate time points.
Develop/initiate/revise the plan of care as necessary and initiate appropriate preventive and rehabilitative nursing procedures.
Provide those services and/or treatments requiring substantial and specialized nursing skills. Provide skilled nursing care to clients utilizing infection control measures that protect both the staff and the client (OSHA)
Coordinate client care, make necessary referrals, report to physician/other staff members changes in client needs, evaluate outcomes of care, and plan for discharge from service. Monitor assigned cases to ensure compliance with the requirements of third party payor.
Provide effective communication/counseling to clients/families, team members, and other health care professionals in meeting nursing and other needs.
Complete appropriate documentation in a timely manner.
Assign the HHA to a specific client.
Supervise and provide clinical direction to HHA/LVN providing services.
Demonstrate commitment, professional growth by participating in in-service programs, and competency.
Promote Agency philosophy and administrative policies.
Perform on-call responsibilities and provide on-call service to clients/families as assigned
