The Commons, located in Minneapolis, is the headquarters of Allina Health, a not-for-profit health care organization serving patients and communities. Approximately 1,600 corporate and administrative service employees work at offices within Midtown Exchange, the site of the historic Sears retail complex at Chicago Avenue and E. Lake Street. This community landmark, adjacent to the campus of Abbott Northwestern Hospital, part of Allina Health, is the second largest building in Minnesota and includes offices, an internationally themed public market and residential units.

Assesses patients on admission to the hospital, and in the hospital setting. Performs and documents history and physical examination based on age and history on complex and chronically-ill patientsRounds on hospitalized patients in various settings including acute care to advance the plan of care, reassesses as patient condition warrants. Updates and records changes in health statusPerform cross-coverage on hospitalized patientsFormulates the appropriate differential diagnoses based on the history, physical exam, and clinical findingsFormulates a plan of care to address complex, acute and chronic health care needs. Integrates knowledge of pathophysiology that pertains to acute and chronically ill patients to assist in planning and implementation of treatmentSynthesizes data from a variety of sources to make clinical judgments and decisions about appropriate recommendations and treatmentsAssesses the impact of an acute, critical and/or chronic illness or injury on the individual's: health status (physical and mental), functional status, including activity and mobility, nutritional status, sleep and rest patterns, quality of life, family, social, and educational relationshipsAssesses patient/family bio/psycho/social needs considering physical and age-specific needs, recognizing the dynamic nature of the patient's condition, and provides psychosocial support to patient and family membersEducates patients/families about self-management of acute and chronic illness with sensitivity to the patient's learning ability and cultural and ethnic background. Collaborates with health care team in ensuring that patient/family needs are metDetermines the need for laboratory and diagnostic studies, initiates orders, and interprets results. Initiates appropriate and timely consultation and/or referral when problem exceeds the nurse practitioner's scope of practice and/or expertiseEmploys appropriate diagnostic and therapeutic interventions and regimens with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacyPromotes collaboration among members ofthe multidisciplinary health care team to facilitate optimal care for complex acute and chronic patientsIncorporates health promotion, health protection and injury prevention measures into the plan of care within the context of the complex acute and chronic illnessPrescribes medication in accordance with practice agreement with the collaborating physician; prescribes appropriate pharmacologic and non-pharmacologic treatment modalitiesAssesses for multiple interactive and synergistic effects of pharmacological agents, including over-the-counter (OTC) preparations and alternative and complementary therapies, in patients with complex acute and chronic illnessFacilitates the patient's transition between and within health care settings, such as admitting, transferring, and discharging patients; coordinates discharge plan and follow-up of patients in collaboration with the physicianSubmits billing levels at time of visit or procedure.Completes consultation, transfer, and discharge summaries as neededMaintains current knowledge regarding state and federal regulations impacting NP practiceUtilizes internal resources (e.g. ethics committee, risk management, legal department) and external resources (e.g. professional organizations, government officials, community agencies) to facilitate the resolution of patient advocacy, moral, and ethical issuesIntegrates the nurse practitioner role into systems, processes, and decision making to function fully within the healthAdapts performance of skills to meet the age and other population specific needs for patients/customers servedDevelops, implements and evaluates the plan of care based upon individual patient and family assessment including age and other specific needs
Education
Master's degree in Nursing required
Qualification/Skills
ExperiencePrefer at least 3-5 years of inpatient or critical care nursing experienceSkillsAbility to work as part of a physician/nurse practitioner dyadExcellent teamworking abilities; professional image via appearance and conductAbility to communicate effectively with patients, staff, physicians, and the publicAdvanced physical and psychosocial health assessment skillsSound knowledge of adult medicine, and related diagnostic and clinical therapiesAbility to teach patients, families, staff, and students; knowledge of research methodologiesCompetent to function as preceptorAble to demonstrate ability to deal with high acuity/complexity and rapidly changing situationsLicensure/ExperienceFormal education as a nurse practitioner in an area consistent with the patient population servedCurrent Registered Nurse License for the State of MinnesotaCertification as a nurse practitioner by appropriate professional organizationMinnesota prescriptive authority and DEA licensureACLS certification preferred
