Job Description

JOB DESCRIPTION: This QDDP (Qualified Developmental Disability Professional)/Risk Manager position is responsible for supporting the Program Director in conducting assessments and quality assurance. This position will perform service coordinating and assessments of adults diagnosed with ID. This position requires knowledge of the DBHDS, Human Rights and DMAS Regulations, confidentiality and storage of records.


ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. All persons in the position must be capable of performing these functions, in adherence to the organization’s policies and procedures.


QDDP Duties and Responsibilities

1. Ensures the timely development of the annual Person-Centered Individual Service Plan (ISP) for each individual supported, in collaboration with case management, family members and other team members and monitor active treatment and documentation.

2. Ensures that the Part five of the PCP ISP is completed, reviewed, being implemented, and coordinating with managers to ensure documentation is being properly completed by staff.

3. Completes all required documentation to ensure the organization is in compliance with DBHDS regulations.

4. Review all individuals’ documentations at least weekly. Utilizing documentation to compile information to prepare quarterly reports on individuals.

5. Coordinate community support and intra-agency support as needed for each individual.

6. Complete the DSP (Direct Support Professional) orientation raining, as well as any other required trainings.

7. Ensure all annual consent forms and necessary yearly medical assessments (tb test, physicals etc.) for the individual is completed.

8. Ability to work varied hours to oversee the implementation of programs and individuals.

9. Assist with developing and presenting the employee’s monthly meetings.

10. Maintain continuing education by attending workshops, seminars and conferences.

11. Develop and revise forms for the program and agency services as needed.

12. Adhere to all of the organization Policy and Procedures.

13. Any additional duties as assigned by the Program Director


Risk Manager Duties and Responsibilities

1. Take DBHDS Approved Trainings and complete training attestation:

  • a. Risk Management
  • b. Individual Risk Screening
  • c. Conducting Investigations
  • d. Root Cause Analysis
  • e. Use of Data to identify risk patterns and trends

2. Coordinate and lead the Quarterly Risk Meetings

3. Analyze data (tracking and trending)

4. Complete investigations for allegations of abuse, neglect and exploitation

5. Complete the annual systemic risk assessment

6. Complete annual safety inspection of each location


Knowledge skills and abilities:

  • Ability to supervise individuals and staff;
  • Ability to problem solve;
  • Ability to utilize conflict resolution strategies and other preventive strategies needed to interpret and respond appropriately to continually changing events;
  •  Ability to influence, negotiate, instruct and guide individuals and staff in a way that is beneficial to the success of everyone;
  • Good decision-making and communication skills are necessary;
  • Good writing/verbal communication skills, in English;


Qualifications:

  •  Must possess Bachelor's Degree preferably in a Human Services field, including but not limited to Nursing, Psychology, Sociology, Social Work and have at least one year experience working directly with individuals who have intellectual disabilities or other developmental disabilities.
  • At least 2 years of supervisory experience.
  • Have a valid driver’s license and a good driving record
  • Training and Other requirements:
  • Completes Orientation topics (within the first 15 days of Hire).
  • Must complete the DSP Supervisory Training, TOVA Training, Human Rights Training
  • Approved Criminal Records Report; Criminal Background Check and Child Protective Services Background check
  • CPR/First Aid Training
  • PPD Testing (TB)
  • Medication Training
  • College transcript / Certificate
  • Strong knowledge of Microsoft Excel and Word preferred.
  • Strong Communication Skills a must.
  • Must be Flexible and reliable


Available shifts and compensation: We have available shifts all days of the week. Compensation is $22.00 - $24.00/hour.
