Job Description

Position Summary: This position is responsible for planning, organizing, developing and directing the overall operation of the company's Nursing Services in accordance with current federal, state and local standards, guidelines and regulations that govern our agency and as may be directed by the Director or authorized federal or state regulatory bodies to ensure that the highest degree of quality care is maintained at all times.

Essential Duties:

  • Responsible for the supervision and oversight of all patient care services including the development of personnel qualifications and the assignment of personnel
  • Manages agency nursing personnel and contracted services
  • Follows up with patients to assess patient satisfaction with the company services
  • Directs in-services, case conferences, and staff meetings
  • Ensures that the Agency operates in compliance with legal, regulatory and accreditation standards
  • Ensures that the policies and procedures reflect quality and safe delivery of home health and home care services
  • Maintains and implements standards of care
  • Represents home health on various committees
  • Assumes budgetary and fiscal responsibility for assigned areas
  • Ensures quality care that is consistent with organizational and regulatory standards
  • Performs home visits and in-services as needed
  • Assumes ultimate responsibility for outcomes related to the performance of the nursing staff and professional development
  • Directly supervises Registered Nurses, Physical Therapists, Occupational Therapists and other skilled care personnel in the field for skilled care services and medically supervised home care in assigned branches
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Abides by all regulations, policies, procedures and standards
  • Performs other duties as assigned

Position Requirements & Competencies:

  • "Clinical Director" means a registered nurse who is sufficiently qualified to provide general supervision and direction of the skilled services offered by the home health agency. The "Clinical Director" and "Director" may be the same individual if that individual is dually qualified. The director shall: (1) Have a Bachelor's Degree with five years healthcare experience and at least one year supervisory experience (full-time or equivalent) in home health care, or (2) Be a registered nurse with five years health care experience AND at least one year of supervisory experience (full-time or equivalent) in home health care, 3-5 years preferred
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state
  • Must possess a valid and unrestricted Driver's License, proof of insurance, and clean driving record
  • HCHB experience and Oasis certification preferred
  • Expertise in skilled care intake process including but not limited to F2F, Physician orders, 485s and Homebound status required
  • Experience leading, developing and growing a skilled care census
  • Must be able to read, write, speak, and understand the English language
  • Must be knowledgeable of nursing/medical practices and procedures, as well as state and federal laws, regulations, and guidelines that pertain to skilled home care

Working conditions: The physical demands described here are representative of those that must be met by an employee to success. The employee frequently is required to reach with hands and arms. The fully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit, talk, hear; use hands to finger, and handle controls. (Must not have any back lifting/moving restrictions). Employee is occasionally required to stand, walk, and stoop. Specific vision abilities required by this job include close vision and the ability to adjust focus.

Environment: Moderate noise level similar to a typical office environment with computers, printers, and work activity.
