Aveanna's broad range of services expands beyond in-home private duty nursing care to include in-home aide services, respite care, school nurse services, therapies, and rehabilitation. Job Responsibilities: Evaluates referred patients accurately using appropriate tests, measurements, and history in the patients' homes or other community affiliations, based on department needs. Develops comprehensive treatment plans in conjunction with the patient and/or family and physician, which is based on the evaluation results and includes the frequency and duration of treatment, long- and short-term goals, expected functional outcomes, and discharge plans.
Administers appropriate therapeutic techniques accurately and continuously monitors the patient's response to treatment, modifying the program as needed. Communicates with appropriate case manager and members of the team Documents patient care based on the agency standard to facilitate continued care and/or billing Demonstrates overall skill in caseload management: and maintains productivity average, scheduling visits efficiently in relation to patient needs. Maintains and enhances professional knowledge and skills through participation in continuing education programs, professional organizations, and/or review of professional literature.
Participates in departmental meetings, projects, and ongoing performance improvement activities. Job Requirements: 1. Massachusetts licensed occupational therapist.
2. One year of experience, preferably in community health. 3.
Excellent written and oral communication skills. 4. Current BCLS certification.
5 . Valid driver's license.
