Patient Care Coordinator

ARJ is looking for a Patient Care Coordinator to provide support and resources to the clinical teams in relation to patient care coordination activities. The Patient Care Coordinator will be responsible for serving as a communication liaison between patients and all members of the intake team for the processing of new referrals, as well as assisting in the maintenance of patient medication profiles.
Responsibilities:
Function as support and resource for the clinical teams related to patient care coordination activities
Serve as a communication liaison between patients and all members of intake team for processing of new referrals
Assist in maintenance of patient medication profile
Perform outbound calls for shipment coordination
Schedule prescriptions to assure the timeliest and most cost-effective delivery methods are utilized
Assist in coordination and communication activities related to patient deliveries
Assist in inventory allocation process for new referrals
Assist in data collection for the prior authorization (PA) process to complete in a timely and appropriate manner
Completes administrative functions for clinical and intake team members including but not limited to: faxing of requests related to medication refills ad medical record requests
Develop and process required reporting on a scheduled and as needed basis
Completes refill phone calls and documentation according to established process
Completes pre and post infusion patient calls and documentation to promote timely medication delivery, care coordination, and adherence to therapy
Triage incoming patient call as needed to ensure timely response and follow-up occur
Completion of documentation related to performance improvement indicators
Maintain confidentiality of patient and proprietary information
Qualifications:
Registered/Certified Pharmacy Technician (CPhT), or as required by the State Board of Pharmacy preferred
Good inter-personal and communication skills
Demonstrated ability to manage multiple priorities and deadlines
Demonstrated proficiency in organizational skills with the ability to work in a fast paced, self-directed environment
Demonstrated proficiency in use of electronic medical records
Demonstrated proficiency with Microsoft Office Suite programs
Minimum of High school diploma or GED required
