Work Location: Tempe, AZ

Pay Rate: $16/hr

Hours:

Predominately Mon-Fri with 1-2 Saturdays per month. (Saturday is an 8:30-5pm shift.)

  • The Mon-Fri is 2 days, 3 evenings
  • Day shift is 8-4:30pm
  • Evening Shift 11:30-8pm

Summary:

  • Responsible for inputting and locating participant information in member management system.
  • May contact participants for scheduling and enrollment information.

Job Responsibilities:

  • Utilize Policy and Procedures for locating participant demographics when participants have been identified as unable to locate or no response
  • Verify participant eligibility and diagnosis verification
  • Input direct referral participant information to member management system
  • Contact participants for initial assessment to identify program intervention level
  • Contact participants for scheduling of Baseline calls and Home Visits
  • Assist Clinical Assistants as necessary.

Education/Experience:

  • High school diploma or equivalent.
  • 0-2 years of customer service experience.

Preferred:

  • Bilingual - Spanish speaker
