Care Manager Registered Nurse

Pay: $40-$45 per hour dependent on experience

Schedule: M-F 8:00am-5pm, no weekends must live near the

• May convert to 10-hour shifts, 4 days a week after orientation
• Remote worker, may have occasional requirement to come into office for staff meetings quarterly
• Must live near Indianapolis or Plainfield, IN

Job Responsibilities:

• Outreach to pregnant members (cold calls) to complete CM program enrollment assessments and create Care Plans

Describe the performance expectations/metrics for this individual and their team:

·        Maintain Case Load 73

·        Daily outreach20 -40 calls daily

·        Monthly Quality Audit score 90%

What previous job titles or background work will in this role?

·        RN license, h/o Prenatal, Labor and Delivery RN, Care Management experience preferred, knowledge of Managed Care insurance activities

Requirements:

• Must have Labor and Delivery RN or Prenatal RN/High Risk OB Nurse experience
• RN License in Indiana
