- Pay: $16.50 +health benefits after 90 days

- Various 8 hour shifts available Monday - Friday: 8am to 4:30pm or 11:30am to 8pm,

- Verify participant eligibility and diagnosis verifications
- Input direct referral participant information to member management system Contact participants for initial assessment to identify program intervention level
- Responsible for inputting and locating participant information in member management system.
- May contact participants for scheduling and enrollment information.
- Utilize Policy and Procedures for locating participant demographics when participants have been identified as unable to locate or no response
- Contact participants for scheduling of Baseline calls and Home Visits Assist Clinical Assistants as necessary..
- Around 1 year of call center experience
- At least 6 months of medical/Healthcare or pharmacy experience required
- GED/HS Diploma REQUIRED
