Job Description

Department: Clinical
Reports to: Counseling Program Manager; Clinical Director


Summary:
Under general supervision, provides case management along with service and resource navigation and
coordination in a mental health human services environment, particularly with veterans, first
responders, frontline healthcare workers and their families.
Essential Duties and Responsibilities include but are not limited to the following:
● Interviews clients to obtain client/family history or other pertinent information.
● Observes clients to detect indications of abnormal physical or mental behavior.
● Plans and administers evidence-based integrated therapies to assist clients in learning to
effectively manage symptoms and other problems.
● Changes method and degree of therapy when indicated by client’s reactions.
● Assists clients in getting substance use screening, assessment and referral to treatment as
needed.
● Discusses progress toward goals with clients
● Consults with other colleagues and/or collaborating professionals concerning treatment plan
and reviews/amends plan consistently.
● Refers clients to supportive services to supplement treatment, counseling, and aftercare.
● Develops evaluative studies of therapy and therapy outcome.
● Complies with all One Tribe Foundation policies.
● Ensures services are client focused as well as ethically and professionally sound.

  • Every Six Sessions
  • Discharge Ongoing assessment of client progress and needs
  • Treatment notes are kept current and up to date for all clients

● Exercises good judgment and demonstrates resourcefulness
● Communicates effectively with others.
● Positively represents One Tribe Foundation in the Community
● Maintains current licensure requirements in good standing
● Maintains required malpractice insurance in good standing
● Balances client requirements with needs to provide accurate results
● Maintains an accurate client status listing to ensure that clients that are complete are properly
discharged to allow room for new clients
● Has the proper judgment to know when to discharge or refer a client
● Continues to meet state requirements for continuing education
● Continually seeking self-improvement and growth through training, education, self-care, and
personal therapy


Education/Experience
● Fully licensed or Associate, LCSW, LPC, LMFT, or Psychologist
● Extensive knowledge of mental health conditions and best practices for psychological treatment
and counseling practices and techniques, including crisis prevention and intervention.
● A minimum of three years of work experience in case manager, service connector, or other
relevant direct practice. Experience with health services a plus.
● Ability to conduct comprehensive, client-centered activities in accordance with best practices.
● Ability to work in a variety of settings with culturally diverse persons and communities with the
ability to be culturally sensitive and appropriate.

Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in
situations where only limited standardization exists. Demonstrates persistence and overcomes

instructions furnished in written, oral, diagram, or schedule form. Displays original thinking and
creativity, generates suggestions for improving work, and develops innovative approaches and ideas.
Displays willingness to make decisions; supports and explains reasoning for decisions.


Mathematical Skills:
Ability to add, subtract, multiply, and divide in all units of measure, using whole
numbers, common fractions, and decimals. Ability to compute rate, ratio, percent and to draw and interpret bar graphs.

 

Computer Skills:

To perform this job successfully, an individual should have knowledge and proficiency in CMS software, MS Office, Adobe. Ability to comfortably communicate via email (Google Suite would be ideal). Ability to create, input export, import and record data in Excel spreadsheets

In 2012, the Veterans’ Administration (VA) released a Suicide Data Report that found an average of 22 veterans die by suicide everyday. The 22KILL initiative started in 2013, at first just as a social media movement to raise awareness, and later became an official 501(c)3 nonprofit organization. 22KILL is committed to researching and understanding the common issues connected with suicide; including Post Traumatic Stress and depression, the various circumstances that they stem from, their effects on the individual, and the impact that suicide has on family, friends, and the community as a whole.
