Job Overview


Visiting Nurse Health System is hiring a Hospice Weekend On-Call RN who provides nursing care, emotional support, and education to terminally ill patients and their families. This role requires clinical expertise in nursing assessment and diagnosis, along with advanced proficiency in technical skills needed to develop, provide, and coordinate patient care. The Weekend On-Call RN serves a key role by triaging patient and family calls, completing admissions, developing plans of care, providing symptom management, and coordinating with interdisciplinary team members such as Chaplains and Social Workers. The RN will handle calls from patients, families, or facilities to assess needs, provide guidance, schedule visits, and ensure compassionate care delivery. Weekend schedules will vary (Friday through Monday or Saturday and Sunday), with exact hours specified in each RNโ€™s formal offer letter.

What Youโ€™ll Do:

  • Provide nursing services as required by your competency checklist, including managing urgent needs at the bedside, administering comfort medications, and delivering emotional and practical support.
  • Evaluate patient responses to care and document thoroughly in visit notes, including patient and family education on disease process, self-care techniques, and prevention strategies.
  • Complete accurate clinical documentation and ensure records comply with physician orders, frequency requirements, and regulatory standards.
  • Obtain and document physician orders for care and revise plans of care as patient conditions change.
  • Assess patients for admission to the hospice program, coordinate care across home and inpatient settings, complete transfer forms, and communicate updates to staff.
  • Make referrals to other agencies on behalf of patients as needed.
  • Participate in on-call responsibilities, providing services to adult and pediatric hospice patients in collaboration with pediatric backup staff.
  • As time allows, conduct record reviews, assist the Hospice Center, and attend weekly home hospice team meetings.
  • Practice and demonstrate infection control standards per VNHS policy.

Youโ€™re Perfect for the Position if you:

  • Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
  • Hold current licensure by the Georgia State Board of Nursing and a valid CPR certification.
  • Have at least one year of experience in critical care, medical-surgical, hospice, oncology, or community health nursing (national certification preferred).
  • Possess a valid driverโ€™s license, reliable transportation, and current auto insurance.
  • Understand the physical, social, and emotional needs of elderly, disabled, and terminally ill patients.
  • Are knowledgeable about Medicare, Medicaid, and funding source regulations as they apply to hospice services.
  • Demonstrate advanced nursing practice skills and can collaborate effectively with interdisciplinary teams.
  • Have strong communication skills, adapting to various audiences and situations.
  • Are proficient with computer basics, including internet, email, electronic documentation, and online HR platforms.
