POSITION SUMMARY
The Registered Nurse (RN) - Weekend Day RN (On-Call) plays a vital role in ensuring timely, high-quality hospice care during weekends, evenings, and holidays. This position provides responsive clinical support and symptom management, helping maintain continuity and excellence of care across all settings.
This role is ideal for an experienced, independent hospice nurse who is passionate about providing compassionate, patient-centered care and can confidently manage on-call responsibilities.
PRIMARY RESPONSIBILITIES

  • Respond promptly to patient and family calls, triaging needs and providing compassionate, timely hospice care.
  • Perform comprehensive nursing assessments including physical, emotional, and psychosocial evaluations.
  • Assess, manage, and alleviate symptoms such as pain, shortness of breath, agitation, nausea, anxiety, and other distressing conditions.
  • Implement and adjust the plan of care in collaboration with the hospice physician and interdisciplinary team to meet changing patient needs.
  • Administer medications and perform prescribed treatments according to hospice protocols and physician orders.
  • Conduct medication reconciliation at each visit to ensure safety, accuracy, and appropriateness.
  • Provide education and guidance to families and caregivers on symptom management, medication administration, and end-of-life comfort measures.
  • Offer emotional and spiritual support to patients and families, maintaining sensitivity to cultural and personal values.
  • Coordinate with the interdisciplinary team (IDG), including social workers, chaplains, CNAs, and volunteers, to ensure seamless and holistic care.
  • Report and communicate changes in condition promptly to the hospice physician and leadership team.
  • Participate in on-call triage, facility coordination, and home visits as needed to maintain continuous care coverage.
  • Ensure that all clinical notes, assessments, interventions, medication updates, and communications are accurately documented in the EMR within 24 hours of the patient encounter.
  • Attend and actively contribute to IDG meetings, staff trainings, and performance improvement initiatives.
  • Uphold all regulatory, compliance, and safety standards in accordance with state and federal hospice guidelines.
QUALIFICATIONS
  • Current Registered Nurse (RN) license in the State of California (required).
  • Minimum 1 year of hospice or palliative care experience (preferred).
  • CHPN certification preferred but not required.
  • Strong clinical assessment and critical thinking skills; able to make autonomous decisions in the field.
  • Excellent communication and empathy when supporting patients and families through end-of-life care.
  • Reliable transportation, valid driver's license, and proof of auto insurance (required).
  • Proficiency with EMR systems and electronic documentation.
  • Bilingual abilities in Spanish, Hmong, or other languages commonly spoken by our patient population are a plus.
WORK SCHEDULE
  • 6:00 AM - 10:00 AM: In-person facility visits (on-site)
  • 10:00 AM - 10:30 AM: Unpaid meal break
  • 10:30 AM - 2:30 PM: Documentation and paperwork (office or home)
  • 2:30 PM - 5:00 PM: On-call availability and response as needed (clock in only if called)
  • Compensation: Guaranteed 8 hours/day, plus additional pay for any calls between 2:30 PM - 5:00 PM
