Job description

Position Purpose

The Clinical Pharmacist-Ambulatory Care II provides evidence-based pharmaceutical care services to optimize medication use and improve patient outcomes. This role supports safe, effective, and cost-efficient pharmacotherapy through:

  • Comprehensive medication management and pharmacotherapy consults
  • Disease State Management and patient education
  • Medication reconciliation
  • Assessment of health and medication-related issues
  • Clinical recommendations that align with the patient’s longitudinal plan of care

Additionally, this position serves as a formally appointed preceptor for the pharmacy residency program or a pharmacy student rotation.

Nature and Scope

As a member of the healthcare team, the Clinical Pharmacist-Ambulatory Care II:

  • Provides pharmaceutical care services through in-person, telephonic, and virtual visits to promote appropriate, safe, and cost-effective medication use.
  • Collaborates with patients, providers, and other healthcare professionals to ensure optimal medication therapy and adherence to prescribed plans of care.
  • Reviews and interprets medication profiles, considering formulary coverage and financial implications to support medication access and affordability.
  • Conducts medication monitoring, disease management counseling, and collaborative drug therapy management. Documents all patient interactions and interventions in the electronic medical record (Epic).
  • Develops and updates patient-specific education materials related to disease states and pharmacotherapy.
  • Analyzes drug utilization trends to identify opportunities for improved outcomes and reduced medication costs across the continuum of care.
  • Communicates effectively with providers, patients, and internal departments using clear and professional verbal and written communication.
  • Demonstrates the ability to work independently and collaboratively within an interdisciplinary team.
  • Maintains working knowledge of government pharmacy programs, including Medicare and Medicaid.
  • Participates in quality improvement initiatives and contributes to organizational performance metrics.
  • Serves as a formally appointed preceptor for pharmacy residency program or student rotation

Education:

Must have working-level knowledge of the English language, including reading, writing, and speaking English. Doctor of Pharmacy Degree from ACPE accredited program is required. Appropriate education to obtain and maintain Registered Pharmacist licensure in the State of Nevada.

Experience:

Applicants are required to have 3 years of ambulatory care pharmacy experience or completion of an ambulatory care-focused Pharmacy Residency program. required.

License(s):

Ability to obtain and maintain a State of Nevada Pharmacist license (status Active or Temporary) required at time of hire.

Certification(s):

A nationally recognized form of advanced credentialing or board certification with regular renewal requirements—such as Board-Certified Ambulatory Care Pharmacist (BCACP) or Board-Certified Pharmacotherapy Specialist (BCPS)—is required at the time of hire.

Ability to obtain and maintain Healthcare Basic Life Support (BLS) certification within the first 90 days of employment is required for all locations.

Computer / Typing:

Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel, and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
