We are looking for a LEAD RN CASE MANAGER for a full-time federal contract (civilian)position supporting the Vermont National Guard (VTARNG) State Medical Detachment, located in Camp Johnson, Colchester, VT 05446. Monday to Friday from 07:30 am to 4:30 pm (weekend availability as needed).

SUMMARY

As Lead RN Case Manager, you'll be at the heart of ensuring service members get the coordinated, high-quality care they need - from complex medical cases to readiness preparation. This role pairs direct clinical case management with project oversight, making it ideal for a nurse who thrives on both patient impact and operational leadership.

MAIN DUTIES:

  • Lead and coordinate case management for service members, from initial identification through medical readiness or separation
  • Coordinate the management of medical records, MEDPROS data, and eCase profiles - ensuring accuracy, completeness, and timely uploads
  • Prepare soldiers and records for MEB/PEB boards; coordinate with commands during monthly case reviews
  • Coordinate treatment plans, referrals, and appointments with military and civilian medical facilities
  • Deliver monthly reports on case status, profile activity, and MEDCHART referral outcomes
  • Oversee contract administrative responsibilities and keep daily operations running smoothly
  • Maintain HIPAA standards and ensure all documentation meets AR 40-66 requirements

REQUIREMENTS:

  • Active, unrestricted RN license in Vermont (or NLC compact license with VT privileges)
  • BSN required; advanced degree a plus
  • Certified Case Manager (CCM) credential highly desirable
  • At least 4 years of nursing experience total, including 2+ years of clinical/case management and 2+ years in a supervisory or team lead role
  • Recent hands-on experience (within 5 years) with military systems: MEDCHART, MEDPROS, or MHS GENESIS
  • U.S. citizen able to pass a Tier 1 background investigation (NACI) for CAC and IT access

Contingent position: This role is contingent on contract award to General Infomatics, Inc., currently under procurement. Selected candidates will be asked to sign a letter of commitment. We'll keep you informed every step of the way and move quickly upon award.
