Job description

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), our Certified Home Health Agency and Licensed Home Care Services Agency.

Our innovative facility and growing services offer opportunities to advance your career. We are currently seeking a full-time Physical Therapist to work for our Certified Home Care Division with experience and will embody the passion and commitment required to deliver the ultimate care for our patients.

Looking to cover Stony Brook to Riverhead and other towns as needed.

Hourly rate is pending credentialing.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • Current New York State Physical Therapy License required.
  • Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
  • A minimum of one year experience as a physical therapist.
  • Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

JOB RESPONSIBILITIES

  • Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
  • Provides assessments, plans and goals.
  • Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
  • Complies with all regulatory criteria.
  • Provides professional leadership and clinical guidance and expertise to team members.
  • Attends regular continuing education programs.
  • Adheres to all aspects of Customer Service, some examples include the following. Working together, showing empathy, courteous communication, respect of others.
