Description
Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems in Indiana. This allows us to work as an extension of our partner hospital systems to service homebound patients in our local communities.
Job Description
As an LPN in home health, you are given the freedom and flexibility to manage your own schedule and provide care one-on-one to adult/geriatric patients in the home setting. A day in the life of the home care nurse consists of providing wound care, infusion therapy, administration and management of medication and delivering education to patients and their families. In a standard work day, a Home Health LPN will typically perform 6 routine visits.
  • The LPN provides direct patient care in the home to adult and/or geriatric patients.
  • Assesses the patient to identify physical, psycho-social, and environmental needs
  • Communicates with the multidisciplinary team regarding patient progress
  • Works under the direction of a Registered Nurse (RN) while demonstrating knowledge and technical skill necessary for providing competent, comprehensive, and age-specific patient care.
  • Implements the plan of care initiated by the RN
  • Provides nursing care within the parameters set forth by the physician's orders and RN assessment to ensure positive patient care outcomes.
  • Documents patient progress and reports any significant changes in a timely manner to the supervising RN.
  • Educates patients concerning procedures, treatments, and needs, and ensures medical orders are carried out accurately and in a timely manner.
  • Provides patients with the knowledge and assistance to learn appropriate self-care techniques and information pertinent to diagnosis and plan of care.
  • Assists in the administration of medications and treatments as prescribed by the physician
  • Assures completeness of ordered procedures and treatments
  • Provides accurate and timely documentation in accordance with the plan of care
  • Participates in personal and professional growth and development
  • Participates in on-call duties for assigned territory
Requirements
Basic Qualifications
  • Must be a graduate of an accredited school of practical nursing and holder of a current permanent or temporary license from the Indiana Medical Licensing Board
  • Possess good verbal and written communication, and organization skills.
  • Must possess an Indiana Driver's license and automobile liability coverage as required by the State of Indiana.
  • Must have the ability to perform essential job functions with or without reasonable accommodation
