Position Overview:



We are seeking a dedicated and detail-oriented Vitals Management Specialist to join our team. In this role, you will be responsible for taking vital signs and monitoring health data for eligible patients at various long-term care and assisted living facilities. You will play a critical role in ensuring that accurate health information is collected and communicated, supporting our remote patient monitoring efforts.

Key Responsibilities:



Vital Signs Monitoring: Conduct daily assessments of patients' vital signs including blood pressure, heart rate, respiratory rate, and temperature in accordance with established protocols.
Data Collection and Reporting: Accurately record and report vital signs data using our electronic health record (EHR) system or other designated tools. Ensure timely submission of data to support remote monitoring and care coordination.
Patient Interaction: Establish a compassionate and professional rapport with patients to ensure their comfort and cooperation with remote patient monitoring program during assessments.
Facility Collaboration: Work closely with facility staff and healthcare providers to coordinate patient monitoring schedules and address any concerns or issues related to vital signs monitoring.
Compliance and Documentation: Adhere to all relevant healthcare regulations and guidelines to maintain patient privacy and data security. Ensure all documentation is completed accurately and in a timely manner.

Qualifications:



Certification: Medical Assistant (MA) certification is required.
