DESCRIPTION:


The Primary Care Behavioral Health Provider will be a core member of the Integrated Primary Care Behavioral Health (PCBH) team. The clinician will provide therapy using evidence-based treatments for adults, children, and families, developing individualized care plans in collaboration with patients and the medical care team.

You will work in a primary care, clinic-based environment - with no after-hours on call, weekends, or major holiday shifts - allowing you to focus on what truly matters: providing quality care and achieving work-life balance.



QUALIFICATIONS:

  • Master's degree in Social Work from a college or university accredited by the Council on Social Work Education, or the Canadian Association for Social Work Education, or foreign degrees determined to be equivalent by the Council on Social Work Education's International Social Work Degree Recognition and Evaluation Service.
  • Current and active Oregon licensure as an CSWA (required to obtain before first day of employment)
  • Demonstrated skill in providing behavioral health assessment, diagnosis, and treatment planning
  • Commitment to compassionate, culturally responsive care

SCHEDULE

  • 5 x 8-hour shifts
  • Days only, Monday through Friday 9-6pm
  • No weekends or on-call requirements
