Job Profile Summary
The Home Health Nurse provides one on one care and skilled services for patients in their own home, contributing to the individual's overall quality of life.? On Call Nurses, primarily perform admissions for all levels of patient care including complex? Infusion and Wound Care. After hours calls that require an in-home visit are completed and telephone education is completed for after-hours calls that do not require an in-home visit. Provides education and resources to patients and caregivers allowing them to manage their conditions and remain in their own home.
Education:
• Required: Graduate of an accredited nursing program
Experience:
• Required: Minimum one (1) year experience as professional nurse within the last three (3) years.
Skills:
• Ability to work effectively, professionally and courteously with co-workers, families, clients, physicians and their staff, and community resources.
• Must be organized and able to exercise attention to detail.
• Ability to adapt/work in fast-paced environment.
• Must be able to work on multiple tasks at one time.
• Ability to provide reliable transportation as required by nature of position.
Licensure/Certification/Registration:
• Required: RN License active in the state of Missouri
• Required: Active Driver's License
