Job description

Job Summary: Registered nurses (or RNs) work closely with supervising physicians to assess and monitor patients’ needs. They ensure that treatment and care are conducted according to the procedures of the facility, as well as administer medications and perform other related tasks.

  • Intake, Care Plan, & Service Agreement 
  • AxisCare Data Input (Care Plans, Service Agreements & Assessments)
  • Clinical Quality Reviews 
  • Supervisory Visits Review
  • Training for Newly hired Staff 
  • Discharge Planning
  • QI Documentation Review 
  • Healthcare Patients/Plans 
  • Change of Conditions Recommendations 
  • Provide palliative care as needed to keep the client comfortable
  • Make recommendations for devices or tools that might improve the client’s quality of life
  • Listen to clients and be prepared to respond to concerns or requests
  • Continuously educate clients, their families and other caretakers on proper home health care strategies and procedures

Requirements:

  • Current nursing license in the state
  • Current CPR (American Heart Association) Certification
  • Access to reliable transportation
  • Ability to work independently and with directions
  • Compassionate and friendly demeanor
  • Experience with Microsoft Office, Excel, Outlook and Word to document and track client care activity

  • Nurses and/or caregivers are trained accordingly to equipment or moderation usage in home 
  • Care Plans are completely Annually at 100% completion rate 
  • Initial care plans are completed in a timely matter as listed in policies and procedures Abuse, neglect on explanation are reported to Administrators 
  • Coordinator services with administrator and Office Manager 
  • Proud Quality Customer Services during the visit 
  • Make recommendations accuracy to change of condition
  • Quarterly quality improvement documentation are complete and reviewed we documented 
  • 100% of staff administering medication are Proxy Trained in every department
