Job description

Nurse-Illinois Hope Houses
Department: Illinois Hope Houses
Reports To: Director of Nursing
FLSA Status: Non-Exempt/.Part-time
Location: Champaign, Illinois
Under the direction of the Director of Nursing (DoN), the Illinois Hope House Nurse provides in-person and virtual medical assessment and case management to clients of One Hope United Hope House (HH) programs. The Illinois Hope House Nurse provides consultation and training to other programs managed by One Hope United (OHU) and potentially oversees, training to staff regarding medication administration, CPR and First Aid, and other medical-related issues. Providing a healthy, safe, clinical, and structured environment, as specified in each youth's Individual Treatment Plan and the OHU Program Plans, is a key component for this role.
Essential Duties and Responsibilities
  • Provides ongoing medical assessment, observation, and oversight of clients' wellness and health.
  • Oversees and facilitates receipt of medication for administration; coordinates with pharmacy; coordinates with other health facilities as youths transition to OHU.
  • Provides triage assessments for youth, care, and/or referral as needed.
  • Provides training to staff regarding medical issues.
  • Available to consult and instruct Hope House staff regarding medication issues, health of clients.
  • Provides education on psychotropic medication regarding use, side effects, storage, and interactions.
  • Available to Hope House youth to consult and instruct regarding physical illness or injury
  • Follows OHU Hope House Program Plan, supporting youths' daily schedules and providing care and treatment to youth that is strength-based, trauma-focused, and person-centered.
  • Demonstrates the highest investment in and commitment to quality improvement practices in order to continuously improve the delivery of services to clients.
  • Participates and contributes to the treatment teams for youths in Hope Houses, communicates with Hope House staff regarding medical issues, and participates in the weekly team. meetings for each Hope House, contributes to individual treatment plan and review meetings as needed.
  • Attends training, seminars, and conferences assigned by the Administrative staff.
  • Completes timely and accurate fashion for all documentation required for the position.
  • Treats with absolute confidence all information concerning youths and programs.
  • Monitors and submits documentation for billing.
  • Maintains records of treatment.
  • Communicates with outside agencies and schools to maintain continuity of care.
  • Performs other related duties as assigned.

Qualifications
Education
  • AA/AS degree in Nursing with a current Illinois license as a Registered Nurse (RN) required.
  • BA/BS degree in Nursing with a current Illinois license as a Registered Nurse preferred.

Experience
  • 3 years of nursing experience.
  • Behavioral/Mental Health setting experience.

Certifications/Licensure
  • Current Illinois Registered Nurse license.

Other Requirements
  • Minimum 21 years of age.
  • DCFS background check & approval to transport youth.
  • Must be physically able to complete Therapeutic Crisis Intervention (TCI) (de-escalation only) training.
  • Driving: A valid driver's license and acceptable driving record are required.

Required Skills
  • Proficient computer skills in a Microsoft Windows environment and adaptive skills to expand software application skills as necessary.
  • Excellent written and verbal communication skills.
  • Patience, positive attitude, and willingness to develop greater skills and understanding of youth with behavior disorders and/or mental health issues.
  • Ability to accept direction and modify interactions with youth at direction of supervisory and clinical team.
  • Ability to coach, manage, and develop team members.
  • Ability to exercise discretion and ensure confidentiality.
