RN Case Manager

The RN Case Manager is a practice-based RN who directly supports Leon Home Health, LLC patients. In collaboration with other members of the healthcare team, the RN Case Manager is responsible for organizing, coordinating, and care management services to patients within the practice.

Responsibilities include:

  • Provides initial and on-going assessment of client needs using the OASIS data elements incorporated into the Comprehensive Assessment.
  • With input from patient/care giver and in conjunction with physician, formulates and implements plan of care.
  • Evaluate effectiveness of care plan and make necessary adjustments.
  • Provides for the emotional and physical comfort and safety of client taking into consideration their rights and cultural background.
  • Receives and transcribes physician orders.
  • Notifies physician and Agency supervisor of unusual reactions and/or changes in client's condition.
  • Documents all appropriate observations and treatments in keeping with Agency policies and procedures.
  • Participates in case conferences, team meetings, staff meetings, and Performance Improvement activities as assigned.
  • Provide any skilled nursing service for which appropriately trained which is prescribed under the plan of care. Prepares clinical progress notes as per agency policies and procedures.
  • Provide 60 day summary of skilled services and client outcomes to physician and Agency supervisor in keeping with Agency policies and procedures.
  • Adheres to all Agency policies and procedures including but not limited to the HIPAA Privacy rule.
  • Maintains strict confidentiality of all patient, employee and Agency information.
  • Other duties as assigned by supervisor and for which (s)he is qualified.
  • Conducts self in professional manner at all times and in all situations.
  • Participates in all agency in-service programs.

Work Experience:

  • Minimum of three (1) years' experience as a licensed, registered nurse.
  • Experience in a supervisory or quality assurance position in acute and/or home health.

Language Skills:

  • Fluent in English and Spanish.
