Job description

JOB TITLE: Certified Home Health Aide - Home Care (Full Time, Days/Evenings)
DEPARTMENT: Home Care
LOCATION: Rochester Regional Health Home Care - Newark, 800 West Miller Street, Newark, NY 14513
HOURS PER WEEK: 40
SCHEDULE: Full Time, Day/Evening Shifts
SUMMARY
As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.
RESPONSIBILITIES
  • Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands
  • Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
  • Documentation. Accurately and completely document client care on activity sheet
  • Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.
Required Qualifications
  • Compassionate, patient focused and dependable professional
  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision
  • Exceptional documentation and communication skills
  • Ability to read, write and speak English, ability to follow verbal and written instructions
  • Valid NYS Driver's License
  • HHA Certified or Ability to meet one of the following eligibility requirements for Competency:

Currently certified CNA along with completion of written test and one day HHA Skills competency class within 4 days of hire.
Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
The Certified HHA who wishes to be employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must complete a written test and pass the one day HHA Skills competency class within 4 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months, and is actively certified, no class is needed.
Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 4 days of hire.
Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 4 days of hire.

PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
