Job description

Haven of Care HomeHealth is seeking a compassionate, organized, and experienced Licensed Practical Nurse (LPN) Care Coordinator to support our growing home health agency. The LPN Care Coordinator serves as a key liaison between patients, caregivers, physicians, referral sources, and agency staff to ensure quality patient care, timely service delivery, and regulatory compliance.
The ideal candidate has home health experience, strong communication skills, and the ability to manage multiple priorities while supporting both clinical and administrative operations.
Key Responsibilities
  • Coordinate home health visits for Skilled Nursing and Home Health Aide services
  • Ensure implementation of physician orders and Plan of Care
  • Monitor delivery of in-home care services and follow up on missed visits
  • Communicate patient updates to RN Case Managers and Clinical Manager
  • Assist with patient intake, referrals, and service scheduling
  • Track physician orders and ensure documentation compliance
  • Coordinate communication between patients, families, caregivers, and providers
  • Support hiring coordination and onboarding of new RNs and HHAs
  • Assist with staff training coordination and compliance tracking
  • Help ensure compliance with home health regulations and agency policies

Qualifications
  • Active Indiana Licensed Practical Nurse (LPN) License
  • Previous Home Health experience required
  • Knowledge of Medicaid Prior Authorization (PA) process strongly preferred
  • Strong coordination, communication, and organizational skills
  • Ability to manage multiple patient care activities
  • Experience with home health EMR systems preferred
