PRN RN Case Manager

What You'll Do:

  • Provide comprehensive care management and coordinate patient care plans across various settings.
  • Serve as the key advocate for your patients, ensuring they receive the best possible care.
  • Conduct patient assessments, develop personalized care plans, and adjust treatment as necessary.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Monitor patient progress and outcomes to continuously improve care quality and patient satisfaction.
  • Educate patients and families on care plans, treatments, and necessary follow-ups.
  • This position allows for work in the field and requires regular weekly travel to client homes throughout WAKE County and surrounding areas.
  • Perform supervisory visits every 90 days (60 days if CAP) for clients in assigned areas.
  • Communicate any changes in client condition with primary nurse.
  • Communicate any concerns regarding aides with field supervisor.
  • Regularly re-evaluates patient nursing needs.

Requirements:

  • Active RN license in North Carolina or have a multi-state license.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Minimum of 2 years of nursing experience (case management experience preferred).
  • Demonstrated ability to read, write, and effectively communicate in English.
  • Strong clinical skills and a deep understanding of case management principles.
  • Excellent communication and patient advocacy skills.
  • Prior home care experience strongly preferred, but not required.
  • A passion for helping others and a compassionate, patient-centered approach to care.
  • Solid computer skills: prior experience with electronic medical records (EMR) preferred.
  • Must have own reliable vehicle.
  • Must be flexible.
  • Ability to travel to cases as assigned.
