Job description

Join our dynamic rehab team as an Evening LPN/RN! Provide compassionate care, support resident recovery, and make a real difference-all during a convenient 6:00pm - 10:00pm shift. Position Summary:

We are seeking a skilled and compassionate LPN or RN to join our evening rehab team. The ideal candidate will provide excellent clinical care, ensure resident safety, and support the rehab team in meeting the needs of residents during the evening hours.


Key Responsibilities:

  • Administer medications and treatments as prescribed by the physician.
  • Monitor, assess, and document resident condition, progress, and response to treatments.
  • Assist in implementing individualized care plans in collaboration with rehab and nursing teams.
  • Communicate effectively with residents, families, and interdisciplinary team members regarding resident needs and concerns.
  • Respond promptly to resident needs and emergencies.
  • Ensure compliance with all facility policies, procedures, and regulatory requirements.
  • Provide support and guidance to rehab aides and other team members as needed.

Qualifications:

  • Current LPN or RN license in the state of Nebraska. 
  • Experience in a rehab or long-term care setting preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Commitment to providing compassionate, resident-centered care.
