Clinical Intake Representative

Full Time, 40 hrs.

Monday -Friday

8am - 5pm

After 6 months- open to a work from home discussion

Essential Functions

Answer incoming calls for Choices Palliative Care with speed and professionalism. Directs calls/messages to appropriate staff as needed.
Process referrals and initiate and maintain patient electronic files.
Process consents, agreements and other forms for signatures and filing, utilizing email, software, and/or mail
Schedule visits as directed by RNs and providers in Outlook and EPIC software.
Handle correspondence and general inquires from patients, families and referral sources.


Qualifications

High School or GED required

1 year of healthcare experience required
