The Greater Manchester Region has approximately 2,500 employees. It includes Manchester Memorial Hospital, a 249-bed community hospital, Rockville General, a campus of Manchester Memorial Hospital, a 102-bed facility, a large multispecialty provider group and visiting nurse services. POSITION SUMMARY:

Utilizing the nursing process, provides high quality direct and indirect patient โ€“centered care.ย  Coordinates patient care by working collaboratively with the interdisciplinary team.ย  Communicates, as appropriate, the patientโ€™s clinical condition, responding quickly and accurately to changes in condition or response to treatment.ย  Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.ย  Promotes the profession of nursing and professional practice through collegial support and interactions. Reports to the Clinical Nurse Manager.

1.ย ย ย ย ย ย  Performs population-specific comprehensive assessments and re-assessments of the patientโ€™s condition and care needs including the psychosocial, physical, educational and cultural aspects.

2.ย ย ย ย ย ย  Demonstrates knowledge of the principles of growth and development of the child to geriatrics and the skills necessary to provide age-appropriate care to patients served. Able to interpret data about the patient status in order to identify each patientโ€™s age-specific needs and provide care needed by the patient group.

3.ย ย ย ย ย ย  Assess and reassess pain.ย  Utilizes appropriate pain management tools and techniques.ย  Educates the patient and family regarding pain management.ย  Documentation reflects assessments, interventions and evaluation of effectiveness.

4.ย ย ย ย ย ย  Develops and revises individualized plans of care as indicated by assessment and patient response to treatment, collaborating with other disciplines as needed.ย  Involves the patient and family in formulation of goals.ย 

5.ย ย ย ย ย ย  Follows the five (5) medication rights and reduces the potential for medication errors. Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition.

6.ย ย ย ย ย ย  Performs all aspects of patient care in an environment that optimizes patient safety and promotes positive patient outcomes.

7.ย ย ย ย ย ย  Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools.

8.ย ย ย ย ย ย  Demonstrates an ability to be flexible, organized and function under stressful situations.ย  Utilizes critical thinking skills and sound judgment in priority setting and delegation.

9.ย ย ย ย ย ย  Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.

10.ย ย  Demonstrates autonomous practice, consistent with evidence-based standards.ย  Pursues personal and professional growth and development.ย  Serves as a professional role model and mentor.

11.ย ย  Provides an accurate and pertinent report of the patientโ€™s condition for safe transfer of care.

13.ย ย  Demonstrates thorough knowledge of proper endoscopy equipment care, including disinfection.

14.ย ย  Demonstrates thorough knowledge of patient care during administration of and recovery from moderation sedation medications.

15.ย ย  Accurately records, handles, labels and processes specimens.


Qualifications

EDUCATION/CERTIFICATION

  • Must be a graduate of an accreditedย school ofย Nursing and have a current Registered Nurse license in the State of Connecticut.
  • Must maintain clinical competency required for area of assignment including the minimum requirement of Basic Life Support and be able to communicate effectively in English, both verbally and in writing.
