Mon-Fri; 8a-430p Home Visits Therapy HCHB charting for visits Physical therapists (PTs) evaluate, diagnose, and treat patients with physical impairments, injuries, or disabilities. Using evidence-based practices to improve mobility, reduce pain, and promote physical well-being. Responsibilities: Conduct comprehensive physical examinations Assess patient's movement patterns, strength, and range of motion Diagnose physical impairments and develop individualized treatment plans Design and administer therapeutic exercises, manual therapy techniques, and modalities (e.g., ultrasound, electrical stimulation) Educate patients on self-care strategies, home exercises, and injury prevention Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists Monitor patient progress, adjust treatment plans as needed, and discharge patients when appropriate Provide patient education on health conditions, treatment options, and lifestyle modifications Qualifications: Master's or Doctoral degree in Physical Therapy from an accredited program National Physical Therapy Examination (NPTE) license State-specific licensure requirements Requirements 1 year PT, home health HCHB experience Benefits Health insurance available upon start of assignment.
