JOB DESCRIPTION

Position: Assisted Living Facility Placement Coordinator

Pay Range: $24.00-$27.00 PER HR

Reporting To: ALF Program Manager

Work Type: Remote (Must live in California)

POSITION SUMMARY:

The ALF Placement Coordinator is responsible for coordinating housing placements for individuals referred to our agency or applying independently for assistance. This position facilitates client assessments, navigates eligibility requirements, and supports the transition of individuals into appropriate assisted living settings. The ALF Placement Coordinator ensures services are client-centered and aligned with state and program-specific guidelines.

QUALIFICATIONS:

  1. 2 years of experience in sales, social services, housing coordination, or elder/disability care preferred.
  2. Preferred Bilingual in Spanish
  3. Possess current CPR certification or can receive upon hiring.
  4. Knowledge of Medi-Cal CalAIM services and California Assisted Living Waiver Programs preferred.
  5. Exercises exceptional judgement and understanding of clinical practices and documentation skills.
  6. Superior organizational skills and ability to work independently.
  7. Good written and verbal communication skills.
  8. Knowledgeable in the use of computers and electronic information and charting systems.
  9. Ability to build and maintain good relationships with Libertana office personnel and RCFE staff.
  10. Familiarity with Title 22

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The following is a representation of the major duties and responsibilities of this position. The agency will make reasonable accommodations to allow otherwise qualified applicants with disabilities to perform essential functions.

  1. Speak with referrals who are interested in the Assisted Living Waiver Program (ALWP), explaining the entire process clearly and compassionately.
  2. Use judgment and decision-making abilities to assess client needs and determine appropriate placement options.
  3. Build and maintain a strong relationship with Assisted Living Facilities (ALFs), their staff, and other relevant vendors.
  4. Identify potential good-fit ALF placements based on client needs and facility availability.
  5. Submit all required documentation in a timely and accurate manner to initiate the placement process.
  6. Notifiy the CalAIM authorization team when individuals are eligible for services.
  7. Manage and update client databases, ensuring accurate and organized records.
  8. Maintain a high level of customer satisfaction and professionalism in service delivery.
  9. Maintain knowledge and compliance with HIPAA, healthcare confidentiality laws, and mandated reporting requirements.
  10. Report any instances of fraud or abuse as required.
  11. Attend all department meetings and State-mandated in-service trainings.
  12. Performs other duties as assigned.

PHYSICAL REQUIREMENTS:

  • Stand, sit, talk, hear, and use of hands and fingers to operate computer, telephone, and keyboard on a frequent basis up to 40% of the time.
  • Reach, stoop, kneel and bend up to 20% of the time
  • Moderate amount of walking up to 15% of the time.
  • Moderate amount of driving up to 25%of the time.
  • Close vision requirements due to computer work on a frequent basis
  • Light to moderate lifting may be required up to 25lbs on a frequent basis.
  • Pushing and pulling up to 25lbs.
