Job description

Earn up to $43/hour | Flexible Scheduling | 1:1 Patient Care 

Monday to Wednesday: 7AM-5PM

Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment. 

Compensation & Benefits 

  • Base Pay: $35/hour  

  • Opportunity to earn up to $43/hour with applicable differentials:  

  • Trach: +$2/hour  

  • Vent: +$3/hour  

  • Nights (7pm–7am): +$2/hour  

  • Weekends: +$3/hour  

  • Holidays: +$3/hour  

  • Weekly pay  

  • Flexible scheduling options (full-time, part-time, PRN)

Key Responsibilities 

  • Deliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines  

  • Monitor patient condition and respond appropriately to changes  

  • Educate patients and families on care plans and procedures  

  • Collaborate with interdisciplinary team members to ensure optimal outcomes  

Qualifications 

  • Active LPN license in Georgia or Nurse Licensure Compact  

  • A minimum of one year of LPN experience required. 

  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization required. 

  • Valid PPD skin test  

Physical Requirements of the Role 

  • Ability to stand or sit for extended periods (up to 12 hours)  

  • Ability to bend, twist, kneel, and assist with patient mobility  

  • Ability to lift, push, or pull up to 50 pounds or more  

  • Manual dexterity for clinical tasks
